As an educator I can tell you no nation has a literacy rate of 100. While North Korea may claim they have that is BS. For example someone with very severe learning difficulties like extreme autism will never be able to read and write, therefore that's why you see many developed nations with 99.9%. Finland always tops league tables for education in Europe and has a literacy rate (according to 2012 data) of 98%.

Though, at higher population counts, the amount of people who are fully unable to become literate is too low to account for even 0.1%, so you could argue it'd be fine to round to a 100% in such a case.
